How do I disable incomplete forms on the dashboard?
There is no way to disable forms on the dashboard. You can only delete them.
Is it possible for clients to update their intake forms rather than fill out a new one?
Patients must complete a new intake form. The new intake form would have some demographic information prefilled.
Can I add a credit card field to an intake form?
No, credit card information cannot be captured on forms. Collecting credit card data through intake forms is not recommended due to security and compliance concerns. To store a card on file, you need to be integrated with Square or Shift4. Clients can add their card when they make a payment or book an appointment.
Can clients update info on their forms rather than having to resubmit a new form?
Clients cannot update information on a form that has already been submitted. If changes are needed, they would have to complete and submit a new form.
Am I able to send a form request by text to a patient?
Noterro does not support sending form requests via text message. You can send forms to patients via email.
Is there a way I can generate a QR code for my intake form that clients can scan?
Noterro does not directly provide a QR code specifically for an intake form.
I want to archive a form that a patient completed?
There is no option to archive completed forms.
Can you turn documents into fillable forms?
You can build intake or consent forms using Noterro’s form templates, but there isn't a feature to upload an existing document (PDF/Word) and auto-convert it into a fillable form.
Can I send the form without a patient being in the calendar?
Yes, you can get the form link and send it to your Patient, or attach the form to your website.
How can I transfer a patient intake form to a new one?
No, it is not possible to transfer answers from a completed client intake form to a new intake form. Clients will need to complete the new form again.
Is there a report that generates a list of people who filled out a specific form?
At the moment, there isn’t a report in Noterro that generates a list of clients who have filled out a specific form.
Can forms be started and saved to come back and complete later?
For intake forms, Partial completion will not be saved, so information needs to be fully filled out and submitted by the patient.
Can I attach intake forms to the patient portal invite email?
The Intake Form can not be attached to the patient portal invite email. You can send Intake forms by email instead. Clients with a Patient Portal account can access forms there.
On the patient intake form, there is a section for insurance information. Is there a way for patients to upload a picture of their insurance card?
Patients cannot upload images, including pictures of their insurance card, during the intake form process. Image uploads by patients are not supported in forms at this time.
Will my clients still get a COVID-19 questionnaire before their appointments?
Clients will automatically receive all necessary intake forms, including any required COVID-19 screening questionnaires, depending on your clinic's specific configuration.
Is it possible to hide the business address from the completed intake form PDF?
No, it's not possible.
