How Do I Complete Outstanding Forms

You might have received a form by email from your clinic, but you can't seem to locate it anymore. All of your unfinished forms will be available for completion once you log in to the App or Portal.

1. Log in to your account 
2. Click on Forms in the Sidebar, or click on Menu in the Footer and then on Forms
3. Click on any of the Forms that need to be completed
