You may choose to collect signatures on forms. First, remember to create an agreement that requires a signature. Then, attach that agreement to your form.
Adding an Agreement
- Log in as the Admin.
- Click on the Gear icon in the top right header.
- Click Templates.
- In the Agreement section, click Add New.
- Add a statement indicating they have read the form (see sample below).
Creating a Form
- Log in as the Admin.
- Click on the Gear icon in the top right header.
- Click Templates.
- Click Add New.
- When creating your Form, Add the Agreement section.
- Select Intake Inform Consent agreement (as from the example above).
