When you are creating your custom form, there are several custom questions you can use to build your form. Below is a list of these questions, and how they will look on a completed form.
Text: This is the text that you can add to your form for instruction or content.
Category: Once you create a category, you can add questions to keep them organized.
Short Answer: This question could be used if you are asking an open-ended question with the expectation of the answer being short.
Long Answer: This question could be used if you are asking an open-ended question with the expectation of the answer being long.
Checkbox: This question could be used if you have a list of items you want someone to select that are relevant to them.
Checkbox with a Comment: This question could be used if you have a list of items you want someone to select that are relevant to them, and that has an additional area for text.
Select One: This question could be used if you want them to select only one answer (multiple choice style of the question).
Drop Down: This question could be used if you want them to select only one answer but is a drop-down menu.
Date: This question could be used if you want to know a specific date of an event (for example, the date of injury).
Range: This question could be used if you want someone to output a response based on a numeric value (for example, what is your pain on a scale of ...).
Insurance Policy Widget: You would use this widget if you want to send someone a form, when completed, would add an Insurance Policy to the profile in Noterro.
Surgeries Widget: You would use this widget if you want to capture information related to surgery.
Medications Widget: You would use this widget if you want to capture information related to medications.
Injuries Widget: You would use this widget if you want to capture information related to injuries.
