You may want to send out a form and agreements at the time of booking, or with the appointment reminder email. You can track forms on the dashboard under Incomplete Forms. If they have not competed the form, you can resend, or copy the link and send it through your email, or have them fill it out in person.
Workflow for Adding COVID-19 Form
- Login as the Admin
- Click on the Gear icon (top right header)
- Under the Calendar heading, click on Services
- At the bottom of the form, check Enable Auto-Form
- Select the Form and
- Click Save
Manual Send Form from an Appointment
- Click on the Appointment in the Calendar
- Click on Add Form
- Select your Form and Agreements
- Under the section " How will your client complete their Form?" select "Send with the Appointment's Email Reminder"
- Click Add Form
The form will remain open until they complete. If they do not complete it at the time of the appointment, you will need to click on Open Agreement for the form to be completed.
