Add a Form to My Website

You can add a Form button to your website by following these steps, and each form template will have a unique link. Please note that every time some clicks on a link on your website, it will create a new patient/client profile in your clinic. This link should be for new patients/clients.

1. Log in as Admin
2. Click the Gear icon (top right)
3. Click on Templates
4. Click on Forms
5. Click on the form Name 
6. Click on the Settings button
7. Copy and paste the Link URL into the appropriate spot on your website. This will allow the form to be shared on your website or email. Upon completion of this form, a new patient will be created 
8. Optionally, you can copy and paste the HTML code into your website’s source code to place a Button on your website
