Creating Form Templates

You can create many types of Forms. The first step is to create Form Templates (for example, Physiotherapy, Chiropractic, Acupuncture, Massage. etc.). There a many options to build your form using Custom Question types.

1. Log in as Admin
2. Click on the Gear icon (top right header)
3. Click on Templates
4. Click on Forms
5. Click on Add New
6. You can add a form from Noterro Templates, from forms you created, or Start with a Blank
