Editing Form Templates

If a form has been completed and saved to a patient/client profile, you will only be able to edit the following.

• Form Status (Control whether or not this form is available to be selected within Noterro)
• Public Access (Allow this form to be shared on your website or emails. Upon completion of this form, a new patient will be created)
• Response Expiry (Select how long your patient/client responses are valid for)

1. Click on the Gear icon (top right header)
2. Click on Templates
3. Click on Forms
4. Click on the Form Name
5. Click on the Settings button
6. You can update the Form Name, Disable the form, Disable/Enable Public Access, and update the Expiry
