Adding a Form from an Appointment

You can add a form directly from an Appointment. If you need someone to update their health information, or if you need to send out a specialized form like a COVID-19 Screening,  you can do it directly from an Appointment. You can also send out agreements with your form.

1. From your calendar, click on the Appointment
2. Click Add Form
3. Select the appropriate Form and Agreements (if applicable)
4. Click Add Form
