Disabling Insurance Questions on the Intake Form

If you do not offer insurance related services, you can disable the insurance related questions by following these steps.

  1. Login as the Admin
  2. Click Gear icon (top right)
  3. Under the Templates heading click Intake Forms
  4. Click on the Edit button of the Intake Form you want to update
  5. Uncheck Insurance Information
  6. Click Save

*If the intake form is saved on a patient/client profile you will not be able to edit the Insurance Information.  You will need to create and save a new intake form.
