Duplicate Intake Form Templates

You may want to make changes to an intake form template, and you do not want to recreate the entire form.  You can simply duplicate the template and make the necessary changes.

1. Click on the Gear icon (top right header)
2. Click on the Black Triangle (next to the Edit button) of the template you want to duplicate
3. Click on Duplicate
4. Make changes to the form (we recommend you change the name) and click Save
