Add/Send an Additional Intake Form to a Patient/Client

You may want to send an additional intake form to a patient/client , or you may want them to update their health history.

  1. Click on the Patient's/Client's name
  2. Click on the Intake Forms tab
  3. Click on Add Intake button
  4. Select the Intake Form and the Agreements (if any)
  5. Choose your method of delivery (Email or Complete Now)
