Creating Intake Form Templates

You can create many types of Intake Forms.  The first step is to create Intake Form Templates (example, Physiotherapy, Chiropractic, Acupuncture, Massage. etc.).

1. Login as Admin
2. Click on the Gear icon (top right header)
3. Under the Template heading click on Intake Forms
4. Click on New Intake Form Template
5. Add a Title
6. Select Sections and Conditions you want to add
7. Click Save
