Managing Agreement Templates

You can manage individual Agreement Templates.  Agreement templates can be used anytime you would need a patient/client to agree to a policy (eg. Privacy Policy), or a procedure (eg. Consent To Treatment). These templates can also include variables like Patient First Name and Patient Last Name.  When creating a new Agreement Template, you can specify the Title, Body, Signature Requirement, and inclusion on the Blank Intake form.  To add a new Agreement Template, follow these steps.

  1. Login as Admin
  2. Click on the Gear icon (top right header)
  3. Under the Template category click Agreements
  4. Click Add Agreement Form
  5. Complete the Form and click Save
